The NYC Annual School Survey deadline

We need 100% Participation.  Please fill out the surveys and either:

•   Drop the self-stamped envelope included inside the green envelope in any USPS mailbox.

•   Have your child return the survey back to school in the self-stamped envelope.

 If you have lost your paper survey, please call the survey hotline at 1-800-690-8603 to obtain your access code and take the survey online.
