Just a reminder for any students reporting to school, families must answer the health screening questionnaire for their children at https://healthscreening.schools.nyc/ before coming to school EVERY day that he/she comes into school.
Families must present proof of this on their phones, devices or print them out before students are allowed in the building. If families are not able to complete the form beforehand, they must then fill out a hardcopy of the form before his/her child(ren) are allowed to come in. In addition, we will have temperature scanners at each entrance to check students' temperatures, too.
Attached is a copy of the questionnaire, along with a Daily Health Screening flyer with directions.
A separate message will be coming about the newest guidance on random testing for students.
